To make a proposal for optimal cancer care in the Netherlands.
Authors' results and conclusions:
In the present scientific report, a committee of the Council drafts a proposal for optimal cancer care in the Netherlands. Demographic changes in the decades to come will bring about an increasing demand on constrained resources. Scientific developments require flexible structures for evaluation and implementation. The report sets out the requirements to be fulfilled by medical professionals and institutions in the provision of adequate cancer care, and its consequences for the allocation of care.
Authors' recommendations:
The committee states that cancer care facilities should be spread geographically when feasible and rational, and centralised when necessary. Basic requirements regard mainly to expertise, infrastructure, professional consultations, evaluation and documentation. Criteria mentioned for concentration are: complexity, rarity, infrastructure, innovation and trends towards subspecialisation. With respect to the needed level of concentration of care, the committee distinguishes three categories of cancer types. Each category is verified against the criteria developed in the report. Implications for practice:Agreements between professionals and hospitals; monitoring of quality within networks.
