[Development of a model for predicting emergency hospital admission and readmission in Catalonia]

Authors' objectives:
The aim of this study was to analyze risk factors and probabilities of unexpected hospitalizations and re-hospitalizations in the population attending primary care centers in an area of Catalonia.
Authors' recommendations:
Predictive models of admissions and readmissions showed factors already found in previous published studies, and also with similar predictive values. In general, age and hospitalizations in the previous year were the factors with more predictive value. Some individual factors not included in the present study would improve the model, such as the inclusion of social support, and morbidity index. In spite of some limitations of the study, models presented here have a potential impact on clinical practice in the attention to individuals at high risk of hospitalization. Besides starting interventions addressed to reduce hospitalizations in these population groups cost effectiveness studies should be also necessary.
