[Total hip and knee artroplasties in Catalonia. Effectiveness and safety]

Authors' objectives: At the international level and especially in Europe, there is an increasing development of population-based registers that aim to analyze clinical effectiveness in medium and long terms of artroplasty surgeries. At the time of the definition of this study, there was no register of this kind in the Spanish National Health System. A retrospective study could help in recommending sources of information, variables and analysis for the set up of an artroplasty register in Catalonia. The aims of this study were to evaluate the characteristics and results of total hip and knee artroplasties (THA and TKA) and propose instruments for evaluating the results of surgery in terms of clinical effectiveness and patient safety.
Authors' recommendations: Factors related to an increased likelihood of postoperative complications and prostheses survival in this project were consistent with published studies based on data from arthroplasty registers or prospective multicenter projects in other countries. This study has allowed to assess the advantages and limitations of sources of information and outcome variables to assess clinical effectiveness and patient safety of THA and TKA in medium and long terms in several hospitals in Catalonia. The analyses carried out will be very useful for application to the data available in the Catalan Arthroplasty Register (RACat). Outcome and adjustment variables have also been identified as relevant to be included in the datasets of this register.
