To summarise the evidence relating to using exercise testing to select which patients with unstable coronary artery disease (UCAD) or after Myocardial Infarction (MI) will be most appropriate for angiography.
Authors' recommendations:
Exercise ECG testing following unstable coronary artery disease or MI is standard practice in many areas. The evidence consists of observational studies of prognostic performance but no studies looking directly at its impact on patient management were found. These studies suggest that the negative predictive value of testing is quite high: that is, patients who have a negative test are unlikely to suffer a further cardiac event and therefore will probably not benefit from angiography. Not all patients with a negative exercise test will avoid angiography. The timing of testing is crucial and clinical flexibility is required.
