Authors' objectives:

This report evaluated the efficacy, safety and cost-effectiveness of pulsatile machine perfusion compared to cold storage in kidney preservation.

Authors' recommendations: The available evidence suggests that machine preservation technology is likely to be cost saving and moreover capital costs are relatively small. The TAU therefore recommends that this technology should be acquired. Since the evidence on which this recommendation is based is far from perfect it is further recommended that transplantation outcomes with machine perfusion should be prospectively recorded and compared with those from kidneys preserved by cold storage. New data from ongoing RCTs may provide additional information on the role of this technology and this report and recommendations will need to be re-evaluated as this new evidence becomes available.
