Authors' objectives:

This report aims to assess the evidence available to assess the usefulness of the DIAM (Device for Intervertebral Assisted Motion) type devices in spinal instability.

Authors' results and conclusions: Only case series studies of poor methodological quality were found, with no control groups and with subjectively reported results. Most case series studies come from a narrative revision of these reports or from the grey literature. Case series studies (the largest found included 912 cases) were found showing good short-term results in terms of pain relief and perioperative complications. No comparative studies were found using other therapeutic alternatives. U.S health agencies consider it, if ever mentioned, as a technology still under investigation.
Authors' recommendations: No good clinical trials comparing results of dynamic stabilization, especially DIAM, with spinal fusion or other therapeutic alternatives have been identified. Nor long term follow-ups with this device have been found. Better evidence is needed to achieve valid conclusions on the efficacy and safety of this type of treatments.
