[Efficacy and safety of hyaluronic acid in the treatment of osteoarthritis of the hip]

Authors' objectives:

Hip osteoarthritis is the most frequent disease of the locomotor system, with radiological changes being observed in 70% of subjects over 50 years of age. It is the most frequent cause of incapacity among the elderly. Hip osteoarthritis or coxarthrosis is more frequent in males under 50 years of age, yet above this age the proportion becomes inverted, with the disease being more frequent in women. Hyaluronic acid (HA) is a mucopolysacharide widely distributed throughout the body. In knee osteoarthritis, it has proved to be as effective as intraarticular corticoids but with a more sustained effect over time. By conducting a systematic review, this study sought to ascertain the efficacy and safety of intraarticular HA in the treatment of hip osteoarthritis.

Authors' results and conclusions: We located 7 clinical trials that met the inclusion criteria and one systematic review. The number of patients in the trials ranged from 22 to 104. Five trials had no control group, one compared two HAs of different molecular weight, and the remainder compared HA with administration of intraarticular glucocorticoids and with a group that received placebo. Relief of pain was estimated to be around 40%-50% by most studies, though the duration of this effect post-treatment was not known. The randomized clinical trial with three arms reported no differences between the treatments at the end of the follow-up period. Moreover, this study displayed the highest quality of all those included.
Authors' recommendations: The absence of a control group in most of the clinical trials means that there is no way of ascertaining the effectiveness of HA in hip osteoarthritis. Accordingly, HA should not be used outside the ambit of experimental studies until better-quality evidence is available.
