Authors' objectives:

The aim of this report is to assess the use of lumbar artificial intervertebral discs.

Authors' recommendations: With regard to lumbar artificial intervertebral discs, the ICSI Technology Assessment Committee finds the following: 1. There are no well-designed controlled trials concerning the long-term (greater than 2 years post-surgery) safety of the lumbar artificial intervertebral disc. Therefore, the long-term safety of the device is unknown at this time. 2. Only a few randomized controlled trials comparing lumbar artificial disk implantation with spinal fusion surgery have been performed. Short follow-up periods, small sample sizes, and outdated surgical techniques were also noted in the studies. Consequently, lumbar artificial intervertebral disc implantation failed to show a clear advantage over other surgical procedures, in particular spinal fusion. 3. No trials were found that compared lumbar artificial disc replacement to intensive multidisciplinary or aggressive non-surgical rehabilitation. Thus, comprehensive comparison of the lumbar artificial intervertebral disc to other treatments has not yet been performed. 4. Overall, the long-term efficacy of the lumbar artificial disc in terms of improving pain rates, disability, range of motion, and quality of life is unknown. As a result, the use of the lumbar artificial intervertebral disc outside the investigational setting is not yet well supported. (Conclusion Grade II).
