Authors' objectives:

This report provides an overview of the use of transmyocardial revascularisation by laser for the treatment of severe coronary artery failure.

Authors' recommendations: NB The following recommendation was developed exclusively for the Chief Executive Officer of the AP-HP, the public hospital network of Paris area, for his use as an aid in the decision making process. In any other context, it should be considered nothing more than an element of information. The CEDIT believes that the mortality associated with the surgical laser revascularisation technique is still heavy and that the benefit for the patient has not been clearly established. The committee is of the opinion that the use of the laser by percutaneous or thoracoscopic route has not yet been sufficiently evaluated. Consequently, the CEDIT has not altered the conclusions of its 1996 recommendation and advises that while awaiting further information the AP-HP should not acquire the laser intended for transmyocardial revascularisation.
