Authors' objectives:
To evaluate mechanical ventricular support systems (VADs) and to estimate the needs of the AP-HP.
Authors' recommendations:
NB The following recommendation was developed exclusively for the Chief Executive Officer of the AP-HP, the public hospital network of Paris area, for his use as an aid in the decision making process. In any other context, it should be considered nothing more than an element of information.
The CEDIT is of the opinion that VAD systems have been sufficiently well evaluated in their indication as a 'bridge' pending transplantation in patients in a state of cardiogenic shock in whom medical treatment has been unsuccessful. In contrast, the extension of their use to other indications cannot yet be recommended without prior evaluation.
The CEDIT therefore recommends:
1. An increase in VAD provision in the two departments which already have some experience of this technology, to meet current needs within the AP-HP; these needs can be estimated to be double the present number.
2. A register should be kept listing the indications for VAD use in these two departments.
