Intraoperative radiation therapy in pelvic gynecologic malignancies

Authors' objectives:

This study aims to summarise the available evidence on intraoperative radiation therapy (IORT) in pelvic gynecologic malignancies.

Authors' results and conclusions: There is still not enough evidence available on the efficacy of this treatment in gynecologic malignancies. Even though there are some series of cases available suggesting a favorable trend as regards historical controls, there are no randomized controlled clinical trials found in the literature determining the usefulness of IORT in this type of tumors.
Authors' recommendations: Based on the currently available information, intraoperative radiation therapy on gynecologic malignancies should be considered only within the framework of protocols and clinical trials which may help to clear up its role. Its extended use cannot be recommended in daily practice. It is thought that the subgroup of patients where the IORT potential benefit could be higher is for those patients with local recurrence involving the pelvic wall.
