Glargine insulin for the treatment of type 1 and type2 diabetes

Authors' objectives:

The aim of this study was to summarise the available evidence on glargine insulin for the treatment of type 1 and type 2 diabetes.

Authors' results and conclusions: Gargline insuline can be a therapeutic alternative for those patients with DBT type 1, specially those who, with one daily NPH injection, present great fluctuations: from an inadequate glycemic control to recurrent symptomatic hypoglycemias. Glargine insulin is not routinely recomended in every DBT type 2 insulin-dependent patient , but it should be considered for: - Those who need help from a third party to administer the insulin injections. - Those who present a significant impairment in the quality of life for recurrent symptomatic hypoglycemias. - Those who need basal insulin twice a day together with oral antidiabetic drugs because they benefit more than those that receive only one basal dose of insulin.
Authors' recommendations: About 14% of diabetic patients would turn to basal insuline therapy with glargine insuline (31% of those who receive insulin), with the potential benefit of controlling their glycemia in the same way or better with one daily injection and with fewer nocturnal hypoglycemias, thus improving their quality of life. However, the lack of information about glargine insulin s long-term effcicacy and safety, and specially its high cost, may limit its use.
