Authors' objectives:

Based on available scientific evidence, the aim of this review was to assess the efficacy/effectiveness of neonatal cystic fibrosis (CF) screening in reducing associated mortality and/or morbidity.

Authors' results and conclusions: Three systematic reviews and a number of subsequently published studies were located. The studies that produced the highest-quality scientific evidence were randomised clinical control trials conducted in the USA and United Kingdom. Outcomes indicated a certain benefit at a pulmonary and nutritional level as a result of early treatment, following detection of the disease through screening. This benefit was only observed in the short term, however, and the studies involved were not free of methodological biases. Insofar as a reduction in CF mortality was concerned, neonatal screening was not observed to have a direct effect on this outcome variable.
Authors' recommendations: Taking the screening principles and quality of the scientific evidence of the above studies into account, we have concluded that neonatal cystic fibrosis screening fails to fulfil all the necessary criteria required for generalised implementation. Although there seems to be some evidence of certain beneficial effects in some parameters, this is nonetheless inadequate to demonstrate the screening programme's efficacy/effectiveness.
