[Neonatal screening of hemoglobinopathies]

Authors' objectives:
Hemoglobinopathies have been defined as the most frequent genetic disorders. They can be divided in 2 different clinical types: structural hemoglobinopathies and thalassemias. The distribution of the disease varies with race and therefore with migrations. The objective of this report was to assess the feasibility of hemoglobinopathies screening in Galicia and the cost-effectiveness of a screening program of this condition in our region. Secondary objectives were to assess if screening is more cost-effective for any type of hemoblobinopathy and to assess the feasibility of screening in population subgroup.
Authors' results and conclusions:
The great majority of studies are about structural hemoglobinopathies and only a few are about thalassemias. The prevalence of drepanocitosis is very high on black population, so many countries have adopted the universal screening of this condition. In Galicia the prevalence is very low. The screening becomes more cost-effective as the prevalence of the risk population increases. The most cost-effective scenario in Galicia would be a selective screening. The newborn screening of thalassemias is not recommended.
Authors' recommendations:
Neonatal screening of structural hemoglobinopathies is effective, but not the screening of thalassemias. Albeit the information on prevalence in Galicia is scarce, this seems to be very low, and the most cost-effective strategy of screening in Galicia would be a selective one.
