Improving the safety of cardiopulmonary bypass duringheart surgery

Authors' objectives:

The objective of this review was to propose French guidelines on the monitoring and safety of cardiopulmonary bypass (CPB) in heart surgery.

Authors' results and conclusions: (i) Technique: CPB is a complex and invasive perioperative technique which requires constant interaction between the anaesthetic, surgical and perfusion teams. (ii) Safety: An American survey found that a CPB incident occurred once every 138 cases and a serious incident once every 1453 cases; there are no data for France. (iii) Impact of monitoring on CPB-related morbidity and mortality: There are no clinical studies. (iv) Practice: A 2003 survey showed that practice varies considerably in France. (v) Guidelines: The report makes 41 grade C recommendations divided into 3 major areas: general conditions and recommendations, safety systems and safety measures, and parameters to be monitored.
Authors' recommendations: Looking ahead: (i) Establish the content of initial training and continuing medical education and define the status of CPB professionals. (ii) Define the respective responsibilities of the health professionals (perfusionists, surgeons, anaesthetists/intensivists) who carry out and monitor CPB and deal with its after-effects.
