This review examines the costs and benefits of gastroplasty.

Authors' recommendations: 1. Gastric surgery is effective in weight reduction in those who are very obese. 2. It appears cost-effective compared to other interventions, mainly because the others are usually ineffective in the long term. 3. The costs are falling with the move to laparoscopic surgery.
