Authors' objectives:

This report aims to assess the effectiveness of modafinil in the treatment of narcolepsy, a rare central nervous system disorder of unknown aetiology characterised by excessive daytime sleepiness (EDS).

Authors' recommendations: This review concludes that: 1. It is recommended that treatment with modafinil should be initiated only by consultants (probably mainly in neurology or respiratory medicine) with expertise in narcolepsy. 2. Further research is necessary, including studies of long-term efficacy, tolerance and side-effects, and direct comparison with other drugs. 3. Patients should be tried on the older and cheaper drugs before modafinil. 4. In order to aid general practitioners in referral of patients with possible narcolepsy, a SIGN guideline should be produced.
