Authors' objectives:
Implantable pulmonary artery pressure telemonitoring aims to detect early haemodynamic deterioration and enable early therapy adjustment in NYHA III heart failure.
Authors' results and conclusions:
High- to moderate-certainty evidence indicates that PAP telemonitoring significantly reduces hospitalisations compared with standard care, with no significant effects on mortality, urgent care, or use of guideline-directed medical therapy. The safety profile is favourable with rare complications (moderate certainty).
Authors' recommendations:
Effects on health-related quality of life remain uncertain due to high risk of bias for patient-reported outcomes. However, real-world effectiveness depends on structured care pathways and adequate resources.

To evaluate the clinical effectiveness and safety, seven studies (3 RCTs, 4 observational studies) were included.
