Mamografía con contraste en el proceso diagnóstico de cáncer de mama
Authors' objectives:
The objective of this report is to evaluate the scientific evidence on
efficacy/effectiveness, safety, economic aspects, as well as women’s values and preferences, environmental, legal, and ethical aspects of CEM in the breast cancer diagnostic process compared to the usual technology in use in the following scenarios:
• Diagnostic scenario 1: Women with dense breasts.
• Diagnostic scenarios 2 and 3: Women with inconclusive findings or with clinical or diagnostic suspicion of breast cancer.
• Diagnostic scenario 4: Women with breast cancer undergoing follow-up after NAT.
• Diagnostic scenario 5: Women at intermediate and high risk of breast cancer undergoing diagnosis.
Authors' results and conclusions:
The evidence compiled in this report suggests that CEM technology shows similar diagnostic performance to other imaging techniques (MG, ECO or CE-MRI), with mild and temporary associated adverse effects. Furthermore, it is a tool that is well accepted by patients and could be a cost-effective option as a complementary technique to MG in some cases. However, the current evidence is insufficient to replace other established imaging techniques across the board. In turn, CEM involves higher doses of radiation, which raises clinical, ethical and environmental concerns, especially in the absence of conclusive benefits.
Future high-quality research could be key to determining in which
clinical contexts CEM can significantly improve the management of breast
cancer, taking into account the need for fully informed consent.
