[Imaging surveillance of a degenerative dilatation of the ascending aorta in adults]

Original Title: Surveillance par imagerie d’une dilatation dégénérative de l’aorte ascendante chez l’adulte
Authors' objectives: In collaboration with the Ministère de la Santé et des Services sociaux (MSSS), the Réseau québécois de cardiologie tertiaire asked the Institut national d’excellence en santé et en services sociaux (INESSS) to develop clinical guidelines regarding the use of medical imaging tests, as well as reasonable and safe imaging frequencies, when monitoring the progression of degenerative dilatation of the ascending thoracic aorta in adults.
Authors' results and conclusions: RESULTS: Briefly, an initial follow-up imaging test is indicated 12 months after the discovery or diagnosis of degenerative dilatation of the ascending aorta. Subsequently, additional follow-up tests may be indicated, with a surveillance schedule tailored to patient characteristics and based on the results of the initial follow-up—namely, aortic diameter, rate of progression of dilatation over time, and the presence of risk factors for complications.
Authors' recommendations: The recommendations resulting from this work specifically target degenerative dilatation of the ascending thoracic aorta, defined as a diameter measuring between 40 mm and 44 mm, or a diameter indexed to body surface area of greater than 22 mm/m². In contrast, an aneurysm is defined as a significant enlargement of the ascending thoracic aorta, with a diameter equal to or greater than 45 mm.
Authors' methods: A rapid review of documents presenting recommendations or clinical indications regarding the subject under evaluation was conducted in accordance with INESSS methodological standards. A consultative committee of primary care clinicians, medical imaging specialists, cardiologists, and cardiac/vascular surgeons was formed to assess the available evidence and to provide contextual and experiential information.
