Authors' objectives:
The main objective of this report is to establish bases for achieving safe and rational use of medicines in surgical areas.
Authors' results and conclusions:
Based on the established selection criteria, the literature search retrieved 35 articles for analysis. Having assessed the evidence provided by the studies, this was divided into four distinct categories (medication general storage, anaesthesia trolley storage, preparation and administration), though recommendations may overlap due to the direct relationships between the categories, since they relate to consecutive steps in the medication-use process, within the surgical process itself.
Conclusions
A total of 138 recommendations were considered, and among these, it was considered important to include in this study 86 practice improvement measures or interventions recommended to avoid medication errors and improve patient safety in surgical areas of hospitals. A lack of published studies of sufficient methodological quality to assess the efficacy of the various strategies meant that it was not possible to rate the strength of the recommendations based on scientific evidence. Priority should be given to addressing medication errors, considering them to be a critical, urgent and widespread public health problem. Systems should be designed to reduce the likelihood of misidentification/incorrect administration of medications with various strategies, using multidisciplinary and innovative approaches to tackle both human and technological factors.
Authors' methods:
To address the objective of the study, we carried out a literature review concerning medication errors and patient safety in surgical areas in the following medical databases: Cochrane Library (Wiley), MEDLINE (PubMed) and Embase (OVID). The search was conducted in November 2018, restricted to studies published in the previous 10 years in English or Spanish. The strategy included, among others, the following terms as free text and controlled vocabulary: medication error, anaesthesia and patient safety. The process was completed with a general Internet search and searches in official websites on patient safety, of the governments of Spain and other countries, as well as websites of relevant organisations, scientific societies, etc., seeking to identify other information of interest. The search was updated in January 2021 to identify recent studies before the final publication of this report.
