Authors' objectives:
To assess whether the use of photon-counting computed tomography (PCCT) compared with conventional CT and other imaging modalities has any impact on clinical benefit.
Authors' results and conclusions:
Evidence regarding clinical benefit of photon-counting CT (PCCT) is limited and does not confirm a clear advantage. PCCT may improve diagnostic accuracy for coronary artery stenosis and influence downstream management, a possible benefit. It may increase detection rates of pancreatic cysts, which is difficult to define as a benefit or a risk. It may also improve classification of breast cancer tumors compared with conventional imaging, a possible benefit. For all other conditions, evidence is lacking regarding its effect on diagnostic reasoning and treatment decisions. Ethical concerns include inequity, poor cost-effectiveness, and resource allocation challenges, underscoring the need for robust research before widespread adoption.
