Original Title: Algorithme de prise en charge de la lombalgie avec ou sans douleur radiculaire
Authors' objectives: Algorithms for managing chronic pain have been developed by interdisciplinary teams at four centres of expertise in chronic pain management associated with the Réseaux universitaires intégrés de santé et de services sociaux (RUISSS) and published on the website of the Ministère de la Santé et des Services sociaux (MSSS). At the request of the MSSS, the Institut national d’excellence en santé et en services sociaux (INESSS) has updated the algorithm for managing low back pain published in 2015.
Authors' results and conclusions: RESULTS : The Web-based format of the algorithm is divided into three main levels: the principal algorithm contains sub-algorithms, with pop-up windows whose content is designed to support achieving certain steps in the trajectory or to provide information on specific concepts and clinical toolkits (level 1), the clinical practice-oriented recommendations adopted in our work (level 2), and the data that support these recommendations (level 3). This document presents only levels 1 and 2. Information on level 3 is available in the Web tool. The main aspects considered in the diagnostic assessment are the predominant type of pain (nociceptive, neuropathic, nociplastic), duration (acute, sub-acute, chronic, recurrent) and biopsychosocial prognostic factors for chronicization. Management of a person diagnosed with low back pain mainly involves non-pharmacological approaches (physical and psychosocial) but may also include pharmacological approaches.
Authors' methods: The algorithm developed as part of this mandate is a decision-support tool bringing together a set of recommendations, along with supporting evidence, regarding the management of non-cancerous low back pain; it is presented in a dynamic Web format. This algorithm was developed in collaboration with clinician and patient partners and is intended to support healthcare professionals practicing in front-line settings. It covers the entire management of low back pain that is non-specific or accompanied by radicular pain in adults, from initial assessment to follow-up and from the acute phase of pain onwards, in order to prevent chronicity
