[Point-of-care ultrasound for the evaluation of pulmonary volume overload in an outpatient setting]

L’échographie pulmonaire portative pour l’évaluation de la surcharge volémique en contexte ambulatoire.
Authors' objectives:
In cardiology and nephrology patients, among others, euvolemia is an imperative objective in their management. It is desirable to be able to reliably detect and quantify pulmonary edema using rapid and, preferably, non-invasive imaging methods. In an outpatient clinic setting such as the Institute’s heart failure and renal failure clinics, lung ultrasound could meet these constraints, if a point-of-care device (i.e., a portable device, as opposed to a stationary device) is used. In this context, the Institute’s Health Technology Assessment Unit (HTAU) was asked to determine the clinical efficacy and safety of point-of-care lung ultrasound for the assessment of volume overload in the context of its outpatient clinics.
Authors' results and conclusions:
The literature search identified 16 publications on the efficacy and safety of point-of-care lung ultrasound for the assessment of volume overload in the outpatient setting in people with heart or kidney failure. No recommendations from evidence-based guidelines and no economic data were identified.
In the context of the outpatient heart failure and kidney failure clinics at the Institute, no recommendations can be made. The paucity of evidence in the outpatient setting and the presence of several neutral outcomes prevent us from clearly demonstrating the benefits that point-of-care lung ultrasound could have compared to conventional physical examination or chest X-ray.
