Authors' objectives:
This study is based on evidence-based science and employs a systematic literature review approach to collect, analyze, and synthesize relevant data. It aims to assess the relative efficacy, safety, and economic impact of Transcatheter Aortic Valve Implantation (TAVI) to support discussions on whether to include TAVI within Taiwan's multi-payment health insurance system. Specifically, the research objectives are as follows:
1. To understand the current reimbursement status in Taiwan and identify appropriate comparators (existing treatments covered by the National Health Insurance).
2. To gather information on reimbursement policies for the assessed technology in other countries.
3. To conduct a systematic literature review of clinical efficacy evaluations for the assessed technology.
4. To conduct a systematic literature review of economic evaluations for the assessed technology.
5. To analyze the current utilization of the technology in Taiwan.
6. To perform an economic evaluation of the assessed technology.
7. To conduct a financial impact analysis of the assessed technology.
Authors' recommendations:
Based on this assessment experience, the study provides several recommendations:
1. The Health Insurance Administration and expert advisory groups should consider selecting appropriate health technology assessment (HTA) evaluation items to improve assessment efficiency.
2. Establish re-evaluation timelines for medical services with insufficient clinical evidence or those still in the early stages of development in Taiwan.
3. Future evaluations of technologies with existing reimbursement precedents should include cost comparisons with similar reimbursed procedures in the application documents. If not proposed by applicants, research teams should collaborate with medical institutions to collect cost, utilization, and clinical outcome data for further discussion.
