[Paclitaxel-coated balloon dilatation in recurrent bulbar urethral strictures]

Original Title: Balón de dilatación con paclitaxel en pacientes con estenosis uretral bulbar recurrente versus dilatación simple o uretrotomía interna visual directa (DVIU)
Authors' recommendations: Moderate-quality evidence shows that paclitaxel-coated balloon dilatation, when compared with endoscopic treatments (simple urethral dilatation/direct visual internal urethrotomy), in patients with recurrent bulbar urethral strictures, would produce a significant net benefit. This technology could result in a fast and sustained improvement in urinary symptoms, voiding function, quality of life parameters, and would reduce stricture recurrences. This was demonstrated by sustained reductions of more than 50% in the International Prostate Symptom Score two years after treatment, increased urinary flow rate, decreased post-void residual urine volume, and patient-reported outcomes. No evidence comparing paclitaxel-eluting balloon dilatation with urethroplasty has been found for recurrent bulbar urethral strictures. The clinical practice guidelines consulted recommend the use of paclitaxel-coated balloon dilatation for recurrent bulbar ureth eligible for urethroplasty The United Kingdom National Health Service (NHS) was the only health funder identified that included urethral drug-coated balloon (Optilume®) within its coverage, as part of the specialized services devices program. No economic studies are available in Argentina or in the region for this technology and its cost-effectiveness could not be determined in Argentina either.
