[Endoscopic full thickness resection of colorectal lesions]

Original Title: Resección endoscópica de espesor total para lesiones colorrectales
Authors' recommendations: Very low-quality evidence obtained from observational studies without control group does not allow to draw conclusions about the risks and benefits of endoscopic full thickness resection using an FTRD® device in patients with colorectal lesions that cannot be lifted after mucosal injection or located in difficult sites. Low-quality evidence obtained from an observational study with one control group does not allow to conclude on the net benefit of endoscopic full thickness resection using an FTRD® device when compared with surgical resection or second conventional endoscopic resection in patients presenting local relapse of the tumor after resection using a conventional endoscopic technique. The clinical practice guideline of the United States gastroenterology societies mentions that further studies are required to better define patient screening and lesion characteristics in order to optimized complete resection rates and safety profile, also considering the oncology data obtained during long-term follow up. The German health system provides reimbursement for this procedure through its policy of third party payer-to-specialized hospitals mediated coverage. The United Kingdom health system does not cover this technology based on the lack of certainty about its safety and inadequate efficacy in terms of quality and quantity. Other coverage policies identified do not mention this technology. Given its low or very low evidence, the net benefit is considered uncertain and no conclusions can be drawn about its cost-effectiveness. There are no economic studies carried out in Argentina or in the region.
