Current perception threshold (CPT) as a quantitative sensory testing (QST) method in hand-arm vibration syndrome (HAVS)

Authors' objectives: To explore the reproducibility, reliability, and validity of CPT for the assessment, diagnosis, and staging of HAVS patients.
Authors' results and conclusions: In total 33 citations were captured with 11 duplicate articles were discarded while 22 articles were examined. Of these 22 articles, five were discarded as earlier articles used CPT for another test “cold provocation thermography” and not “current perception threshold”. Three more were excluded thus a total of 14 studies were examined for this review. Of these, 10 were determined to use other test/outcome measurements and not CPT and were subsequently dropped. Of the four remaining studies, level of evidence was 4 for the three cross-sectional studies we found and the one systematic review we located did not include any higher quality studies. At present, there are no studies that validated CPT as one single QST method to diagnose or clinically stage HAVS. When assessing neurosensory signs and symptoms in workers exposed to hand-transmitted vibration CPT must be used in conjunction with nerve conduction studies (NCSs) and the Stockholm Workshop Scale sensorineural (SSN) component.
Authors' methods: A comprehensive and systematic literature search was conducted on November 28, 2023. The search was done on commercial and non-commercial medical literature databases. A combination of keywords was employed on this search. No limitation, such as on the language or date of publication, was implemented in this search. A manual search was also done on the references of the articles that were retrieved in full.
