[Systematic review of the effectiveness and cost-effectiveness of psychoeducational interventions in people with bipolar disorder and their families]

Revisión sistemática de la efectividad y coste-efectividad de las intervenciones psicoeducativas en las personas con trastorno bipolar y en sus familias
Authors' objectives:
1) Systematic review (SR) of the literature to examine the benefits of psychoeducation programs combined with pharmacological 2) SR to assess the effect of the psychoeducation program received by patients with BD and their families.
Authors' results and conclusions:
RESULTS: A total of 2685 references were located in the electronic databases. Following the duplicates elimination, 1315 references were reviewed, of which 77 were selected based on title and abstract. From reading these references’ complete text, 28 studies were included (corresponding to 33 articles). It was also included a SR (Justo, 2007) that, even when it was published two months later from the search period established, it provides a special interest for the topic that concerns this review. Therefore, there were finally included 29 studies (34 articles): eight SRs (Colom 1998; Reinares 2002; Colom 2004; Vieta 2004; Colom 2005; Justo 2007; Rouget 2007; Scott 2007), twelve RCTs (Glick 1993; Perry 1999; Simoneau 1999; Miklowitz 2000, 2003; Simon 2002, 2005 y 2006; Colom 2003a; Colom 2003b, 2004 y 2005; Rea 2003; Miller 2004; Reinares 2004; Bauer 2006; Miklowitz 2007), a NRCT (Honig 1997), other six CSs (Van Gent 1993; Miklowitz 2003; Kim 2004; Michalak 2005; Benhard 2006; de Andrés 2006) and two QSs (Pollack 1995; Sajatovic 2005). In this SR, all included studies used psychoeducation within their interventions. CONCLUSIONS: Currently the therapeutic interventions used most frequently in BD, such as therapy focused on the family, interpersonal therapy and social rhythm, and cognitive-behaviour therapy, or interventions focused on patients (Perry 1999; Simon 2002; Colom 2003, 2004, 2005; Simon 2005 Bauer 2006 Simon 2006), or as interventions targeted in families (Glick 1993 Simoneau 1999 Miklowitz 2000, 2003, 2007; Miller 2004), all include psychoeducation as a programme’s aim. According to Miklowitz study (2007) there were no statistically significant differences between psychotherapy modalities in relation to their effectiveness in the BD. Psychoeducation allows patients to accept their disease understanding the following basic aspects: their nature, biological and genetic causes, triggers psychosocial factors, prognosis and treatment. Other basic aspects addressed by psychoeducation are drug treatment’s adherence and relapse prevention, because of the inclusion in the programme of the early identification of decompensation signs’. From all this data, we could say that psychoeducation, far from being one more option, it should constitute an essential element in addressing the BD.
