[Systematic review of utilities obtained from Spanish population with cancer, diabetes and heart problems]

Original Title: Revisión sistemática de utilidades obtenidas de población espańola con cáncer, diabetes y problemas de corazón
Authors' objectives: • To review the scientific literature about utilities and preferences for health states in Spanish population with cancer, diabetes and heart diseases. • To develop a database with relevant information about the studies evaluating utilities and preferences for health states in the systematic review.
Authors' results and conclusions: RESULTS: A total of 2592 references were identified. After discarding duplicates, 1527 references were screened. In total, 135 full articles were retrieved and only 9 were finally included. Among these, 4 studies included patients with heart problems and 2 included diabetic patients; other 3 studies included general population and analysed subgroups with diabetes, heart problems and cancer. From these 9 studies 29 utility values were extracted. A large number of full studies finally excluded assessed quality of life using validated questionnaires but the authors did not report utility values, only a descriptive analyses of the quality of life. CONCLUSIONS: • The study of utilities in Spain does not seem to be much extended given that only 29 utilities related to the 3 more prevalent diseases in our country were identified: 20 utilities in heart diseases’ states, 7 in diabetes and 2 in health states related to cancer. • The EQ-5D questionnaire seems to be the most used tool to elicit utilities from the population in Spain. • It is posible and desirable to get a database of utilities elicited from the Spanish population. This database can be accessed by the scientific community for the development of economic evaluations of healthcare technologies in Spain. • To contact experts and studies’ authors is absolutely necessary in order to have a comprehensive database.
Authors' methods: A systematic review of literature was carried out. The search strategy was designed combining terms on utilities (e.g. utilities, eq-5d, hui, etc.) and on Spain as the study context (e.g. spain, espana, espagna, etc.). The databases searched were: MEDLINE, MEDLINE in process, National Health Service Economic Evaluation Database (NHS EED), Database of Abstracts of Reviews of Effectiveness (DARE), Health Technology Assessment (HTA) database, Embase, PsycINFO, CINAHL, Cochrane Central Register of Controlled Trials (CENTRAL), CEA Registry, Índice Bibliográfico Español en Ciencias de la Salud (IBECS) and Índice Médico Español (IME). Date and language limits were not applied. The list of references of included studies was also screened. Several study designs were selected: experimental studies, observational studies, studies validating quality of life instruments, surveys, economic evaluations. The included population was defined as resident population in Spain, any age and sex, who could refer to either general population or population affected by any disease or condition, under treatment or not. This review focuses in three areas: cancer, heart and diabetes. For a study to be included, utilities reported had to be valued through a tool validated in Spain (i.e. EQ-5D, HUI, SF-6D) and/or following accepted techniques (e.g. time trade-off and standard gamble). Two reviewers selected the studies. The data extraction was carried out by one reviewer and checked by a second reviewer. Where the reviewers disagreed, an agreement was achieved with the involvement of a third reviewer whenever needed. The data were included in a spreadsheet template designed ad hoc. The data collected were synthesized through narrative procedures using detailed tables. The data were included later in the first version of the database utilities elicited from the Spanish population.
