Original Title: Revisión sistemática de utilidades o preferencias por los estados de salud obtenidas de población española
Authors' objectives: • To review the scientific literature about utilities and preferences for health states in Spanish population. • To develop a database with relevant information about the studies evaluating utilities and preferences for health states in the systematic review.
Authors' results and conclusions: RESULTS: A total of 2592 references were identified. After discarding duplicates, 1527 references were screened. In total, 834 full articles were retrieved and only 99 were finally included. From these 99 studies 631 utility values were extracted. The greater number of health state values included in the review are related with diseases of the nervous system, digestive system, mental disorders, musculoskeletal system, circulatory system and respiratory system. Few authors have studied utilities related to endocrine diseases, cancer or rare diseases. The EQ-5D questionnaire is the most used tool to elicit utilities. A large number of studies finally excluded assessed quality of life using validated questionnaires but the authors did not report utility values, only a descriptive analyses of the quality of life. CONCLUSIONS: • The review identified 99 papers and 631 utilities for different health states in Spanish population. • The EQ-5D questionnaire is the most used tool to elicit utilities from the population in Spain. A large number of authors used the EQ-5D as a tool to assess health related quality of life without providing an estimation of utilities. • The diseases of the nervous system, the digestive system and the mental disorders are the three groups with the greater number of utilities identified. Few papers studied utilities in cancer or rare diseases. • The vague definition of certain health states limits the usefulness of some utilities for future economic models. • It is necessary to contact experts and studies’ authors in order to have a comprehensive database of utilities elicited from Spanish population.
Authors' methods: A systematic review of literature was carried out. The search strategy was designed combining terms on utilities (e.g. utilities, eq-5d, hui, etc.) and on Spain as the study context (e.g. spain, espana, espagna, etc.). The databases searched were: MEDLINE, MEDLINE in process, National Health Service Economic Evaluation Database (NHS EED), Database of Abstracts of Reviews of Effectiveness (DARE), Health Technology Assessment (HTA) database, Embase, PsycINFO, CINAHL, Cochrane Central Register of Controlled Trials (CENTRAL), CEA Registry, Índice Bibliográfico Español en Ciencias de la Salud (IBECS) and Índice Médico Español (IME). Date and language limits were not applied. The list of references of included studies was also screened. Several study designs were selected: experimental studies, observational studies, studies validating quality of life instruments, surveys and economic evaluations. The included population was defined as resident population in Spain, any age and sex, who could refer to either general population or population affected by any disease or condition, under treatment or not. For a study to be included, utilities reported had to be valued through a tool validated in Spain (i.e. EQ-5D, HUI, SF-6D) and/or following accepted techniques (e.g. time trade-off and standard gamble). Two reviewers selected the studies. The data extraction was carried out by one reviewer and checked by a second reviewer. Where the reviewers disagreed, an agreement was achieved with the involvement of a third reviewer whenever needed. The data were included in a database. The data collected were synthesized through narrative procedures using tables.
