[Use of biomaterials in tympanoplasty for tympanic membrane repair]

Original Title: Timpanoplastia con biomateriales en la reparación timpánica.
Authors' recommendations: "Conclusions Low-quality evidence suggests that the use of platelet-rich fibrin (PRF) in tympanoplasty for tympanic membrane repair may yield a significant net benefit, when compared with tympanoplasty without PRF, as it improves closure rates and reduces the incidence of postoperative infections. However, no significant improvement in hearing has been observed. Low-quality evidence suggest that the use of platelet-rich plasma (PRP) in tympanoplasty for tympanic membrane repair may yield a lower net benefit when compared with tympanoplasty without PRP since, despite increasing rates of closure, did not show significant improvement in hearing and there are no conclusive data on having reduced complications. Very-low-quality evidence does not allow to draw conclusions on the effectiveness or safety of adding fibrin sealants to tympanoplasty, versus not adding them. As regards coverage policies from high-income countries, there is no reference to adding PRF or fibrin sealants to tympanoplasty. Also, some policies mention that PRF is considered experimental. It is worth to note that in Latin American countries, these approaches are not mentioned in their current policies. Additionally, the clinical practice guidelines do not include recommendations for the use of PRF, PRP or sealants in tympanoplasty. As regards the economic component, no cost-effectiveness studies or budget impact analysis assessing the use of tympanoplasty with PRF, PRP and/or fibrin sealants in tympanic membrane repair, have been found either in Argentina or in any other country. Consequently, this component is uncertain."
