Original Title: Réadaptation pulmonaire chez les personnes atteintes de la maladie pulmonaire obstructive chronique (note de synthèse)
Authors' objectives: Chronic Obstructive Pulmonary Disease (COPD) is a respiratory disease often caused by smoking. Pulmonary rehabilitation is an intervention that can substantially improve the quality of life, dyspnea, and exercise endurance in people who have it. However, at the Quebec Heart and Lung Institute – Laval University (Institute), the human, material, and financial resources of the Cardiac Disease Prevention Pavilion limit the capacity to receive new users. In order to support a request for funding that would improve access to the rehabilitation program, the Institute’s Health Technology Assessment Unit (HTAU) was asked to summarize the evidence related to the safety, efficacy, and cost effectiveness of pulmonary rehabilitation in the management of COPD.
Authors' results and conclusions: The literature review identified 10 evidence-based clinical practice guidelines and 28 systematic reviews. The evidence reviewed suggested that pulmonary rehabilitation performed in a centre, at home, or through telerehabilitation is safe and associated with clinical benefits. Overall, the observed improvements are moderately large and clinically significant. It would also be cost-effective by current cost effectiveness standards. However, there remains uncertainty regarding the essential components of pulmonary rehabilitation, such as the optimal duration per session, the ideal delivery environment, the level of supervision required, the ideal intensity, and the duration of treatment effectiveness.
