Original Title: Réadaptation respiratoire à la suite d'une hospitalisation en lien avec un diagnostic de la COVID-19
Authors' objectives: COVID-19 is a disease caused by the SARS-CoV-2 virus, which causes respiratory impairment, but can also be associated with other neurocognitive, cardiovascular and digestive deficiencies. This respiratory disease is manifested by pneumonia, which can progress to a serious condition similar to acute respiratory distress syndrome requiring intensive care. Some professional organizations recommend that patients who survived COVID-19 be systematically assessed in the weeks following their discharge from hospital and assessed for needs related to respiratory rehabilitation. In order to develop a post-COVID-19 respiratory rehabilitation program, the Multidisciplinary Services Directorate at the Quebec Heart and Lung Institute – Laval University (Institute) requested that the Health Technology Assessment Unit determine the most effective health interventions methods for respiratory rehabilitation.
Authors' results and conclusions: The analysis of the available evidence suggests that respiratory rehabilitation and telerehabilitation are safe and effective interventions, based on a level of evidence coming mainly from studies carried out in the context of respiratory diseases other than COVID-19. Respiratory rehabilitation programs should be individualized to include exercises that take into account the patient’s characteristics and needs, and include documentation monitoring the patient’s progress. The practice of respiratory rehabilitation in patients with respiratory problems resulting from COVID-19 is accompanied with a high level of uncertainty. Due to the limited number of studies that were carried out in this population, the recommendations listed in the practice guidelines and expert consensuses are mainly based on expert opinions.
Authors' recommendations: It is recommended that the Institute develop a respiratory rehabilitation program for patients suffering from respiratory problems following a hospitalization related to a diagnosis of COVID-19. This program should include an optional telerehabilitation component, giving consideration to the patient’s needs. It is also recommended that the Institute conduct a prospective study to assess the impact of the respiratory rehabilitation program on eligible clients.
