Original Title: Proposition d’organisation de la médecine bariatrique pour les territoires du réseau universitaire intégré en santé et services sociaux de l’Université Laval
Authors' objectives: The Quebec Heart and Lung Institute – Laval University (Institute) is recognized by Quebec’s Ministry of Health and Social Services as a centre of excellence in bariatric surgery since 2009. Nearly 70% of users who have undergone bariatric surgery at the Institute come from outside the Capitale-Nationale region, despite the availability of bariatric surgery centres in the majority of other administrative regions. The postoperative follow-up of these patients is added to an existing cohort of more than 9,000 patients already followed by the Institute. Faced with the absence of a structured network in bariatric surgery, it was proposed to carry out an evaluation project on the management of prioritization of patients with morbid obesity.
Authors' results and conclusions: With the collaboration of the Integrated University Health and Social Services Network of Laval University, institutions involved in bariatric surgery programs, the working committee, and data from the literature review, the classification and designation criteria for centres offering an integrated bariatric medicine program were determined. Roles and responsibilities according to institutional designations and their level of care were defined. Also proposed, was an evidence-based care pathway between the levels of access to services, including referral mechanisms to support the principle of prioritization of care and services.
Authors' recommendations: It is recommended that the network’s institutions support and participate in the implementation of a system structured around the roles and responsibilities of institutional designations and their level of care, and to collaborate in order to put in place the proposed care pathway according to the levels of access to services and the prioritization of establishments in the territory.
