[Evaluation of the effectiveness and cost effectiveness of cardiac rehabilitation programs]

Authors' objectives: Over the years, advances in treatments for cardiovascular disease have led to improve survival rates following cardiovascular events. The prevalence of cardiovascular diseases remains high in developed regions, generating high costs for health care systems. Cardiac rehabilitation is a secondary and tertiary prevention strategy aimed at reducing the recurrence of cardiovascular events. At the Quebec Heart and Lung Institute – Laval University (Institute), the Cardiac Disease Prevention Pavilion (CDPP) offers a cardiac rehabilitation program thanks to a specialized team of clinical nurses, cardiologists, kinesiologists, and nutritionists. However, the human, material, and financial resources of the CDPP limit the capacity to welcome new users. The Institute’s Multidisciplinary Services Directorate requested the Health Technology Assessment Unit (HTAU) assess the effectiveness and cost effectiveness of cardiac rehabilitation programs, in the context of a funding request to Quebec’s Ministry of Health and Social Services with the intention of maintaining programming at the CDPP.
Authors' results and conclusions: Analysis of the available evidence suggests that cardiac rehabilitation is associated with a reduced risk of hospitalizations, myocardial infarction, and possibly the risk of cardiovascular mortality. Evidence also suggests that cardiac rehabilitation programs are associated with improved quality of life compared to usual care. Other components also play an important role in patient care, such as adherence to the program and psychosocial care. According to economic analyses, cardiac rehabilitation is cost-effective when compared to usual care.
Authors' recommendations: According to the current state of knowledge, it is recommended that the Institute continue the cardiac rehabilitation activities of the CDPP, implement measures to improve accessibility, and facilitate the registration of patients eligible for the program. As part of a process of continuous quality improvement of care and services, it is also suggested that the Institute set up a procedure favouring the systematic referral of eligible clients, and improve monitoring of performance and quality indicators for cardiac rehabilitation activities.
