[Blood conservation strategies in cardiac surgery: the thromboelastogram and rotational thromboelastometer for monitoring hemostasis in cardiac surgery]

Original Title: Stratégies de conservation sanguine en chirurgie cardiaque: le thromboélastogramme et le thromboélastomètre rotatoire pour la surveillance de l’hémostase en chirurgie cardiaque
Authors' objectives: Thromboelastograms are blood viscosity measurement devices used for monitoring blood clotting. While they are available at the Quebec Heart and Lung Institute – Laval University (Institute) to guide the use of blood products, their use in cardiac surgery is not standardized.
Authors' results and conclusions: The results of this review suggest that, compared with conventional laboratory testing, the use of thromboelastography, as part of a transfusion algorithm for monitoring hemostasis in cardiac surgery, is associated with a reduced risk for transfusion of packed red blood cells, platelet concentrates, and fresh frozen plasma. In addition, this strategy is associated with a reduction in the volume of blood products administered, with the exception of fibrinogen. Moreover, the contribution of the devices is indistinguishable from the overall effect linked to the standardization of practices carried out by the use of a transfusion algorithm. Although no direct comparison was performed, the available data do not suggest a difference in the safety and effectiveness of the devices currently available.
Authors' recommendations: Based on these results, the Institute’s Health Technology Assessment Committee recommends developing and implementing a transfusion algorithm to monitor hemostasis in cardiac surgery and recommends the continued use of thromboelastograms to guide the use of blood transfusions.
