Doppler ultrasonography in peripheral arterial occlusive disease of the lower limbs

Authors' objectives:

To assess the power of Doppler ultrasound (DU) in the diagnosis and treatment of peripheral arterial occlusive disease (PAOD), and to define its place as a decision-making aid in treatment, and as an aid to performing bypass surgery and follow-up after bypass and intraluminal revascularisation procedures.

Authors' results and conclusions: Studies selected: - comparative studies of DU and arteriography since 1975 (27 studies selected out of 69); - guidelines and literature reviews on DU or management of PAOD (n=36) since 1975; - reproducibility studies of angiography (6/86) since 1985; - studies on the use of DU for monitoring vascular procedures (68/216) since 1995. Indications: DU performed in addition to a clinical examination and measurement of ankle-brachial systolic pressure index (ABI) is sufficient in most cases to confirm a diagnosis of PAOD, to decide whether the disease should be treated medically or by revascularisation, and for subsequent follow-up. At present, this is the first-choice examination for patients with PAOD. Arteriography is still performed before treatment if it is decided to use revascularisation, and more rarely for diagnosis, if DU is not conclusive. Diagnostic power: The sensitivity of DU in detecting stenosis of >50% is 63-95%, with a specificity of 85-100%. In comparison with arteriography, the gold standard examination, DU predicts whether angioplasty is feasible in more than 80% of cases.
Authors' recommendations: Studies of the benefits of a DU/MRI-angiography combination or a DU/CT-angiography combination, as compared with these investigations performed alone, would be useful. Such studies would provide information for the iliac and femoropopliteal procedures.
