Authors' objectives:
This review consists of a clinical and economic evaluation of implantable cardiac defibrillators (ICDs), comparing them with drug therapies in the treatment of malignant ventricular arrhythmias. This report updates a 1997 ANAES report.
Authors' recommendations:
From this literature review it can be concluded that ICDs are usually superior and never inferior to drug therapies with regard to global survival in the indications that have been evaluated in primary and secondary prevention.
In primary prevention, relevant clinical risk markers (such as ventricular dysfunction) and electrical risk markers have yet to be defined.
The indications and procedures for ICD insertion have now been validated both internationally and in France (Guidelines published by the French Cardiology Society). However, while insertion procedures are well-defined, ICD tariffs and reimbursement procedures are not. This paradoxical situation may partly explain the low number of insertions carried out to date in France.
