[Hypoglossal nerve stimulation systems for treatment of obstructive sleep apnea]

Original Title: Sistemas de estimulación del nervio hipogloso para el tratamiento de la apnea obstructiva del sueño
Authors' objectives: This report aims to provide the National Health System (NHS) with rigorous information on the effectiveness and safety of HNS devices in patients with moderate to severe OSA presenting inadequate adherence or treatment failure with CPAP or other non-invasive treatments, in order to assist the decision-making process on the inclusion of HNS into the portfolio of services provided by our NHS.
Authors' results and conclusions: The comparative effectiveness among HNS devices versus no treatment is based on only one small and low-quality RCT involving the Inspire® Upper Airway Stimulation System. This study was conducted with a group of selected patients, those who positively responded to that particular device. Thus, the results are not applicable to the whole population of interest. This study reported a significant worsening in such outcomes as AHI, ODI and CT90 when the device was disconnected for one week. Likewise, the study reported a significant worsening of quality of life as measured by the ESS and FOSQ scores. The quality of the evidence on the safety of HNS devices proved to be very low. Since the RCT did not provide evidence on safety outcomes, the available evidence does not allow for any conclusions to be made as to whether HNS is safer than no treatment in the population of interest
