[Effectiveness and safety of endovascular treatment with venous adhesive in varicose veins of the lower extremities]

Original Title: Efectividad y seguridad del tratamiento endovascular con adhesivo venoso en varices de extremidades inferiores. Ministerio de Sanidad, Servicios Sociales e Igualdad
Authors' objectives: The objective of this report is to analyse effectiveness and safety of endovenous ablation of varicose saphenous veins with cyanoacrylate closure system in patients with moderate to severe chronic venous insufficiency compared to surgical procedures, as of saphenous vein stripping, and other endovenous ablation techniques.
Authors' results and conclusions: There is low evidence on effectiveness and safety of intravenous sealing with cyanoacrylates treatment for Chronic Venous Insufficiency. Most studies are case series with a low level of evidence quality, and only 2 studies compare the CAC intervention with another treatment. The comparative studies include an RCT and a non-randomised clinical trial, which was assessed with the GRADE tool and it showed a moderate or low level of quality of evidence. Most studies have an average follow-up time less than or equal to one year.  Regarding effectiveness, results are initially positive. The occlusion rate after CAC treatment is 97%, the recanalisation rate is 4.5% and the quality of life improves after the intervention in all patients. The mean time of intervention, the time to total recovery, the need for anaesthesia and the need for compressive bandages seem to be equal or more favourable than in regular interventions for CVI treatment In relation to safety, the procedure has a low or null rate of Deep Vein Thrombosis and other serious adverse effects such as cerebrovascular complications or death. The rest of the adverse effects (phlebitis, ecchymosis, paraesthesia, pain, etc.) are reported in similar or lower rates than those of other reference treatments (EVLA, RFA or surgery).
