Authors' recommendations:
No evidence showing that the use of video head impulse test may improve the diagnosis of patients with vertigo.
Moderate-quality evidence suggests that video head impulse test might have a marginal clinical benefit in the diagnostic algorithm of patients with vertigo, and a negative clinical (non-video assisted) head impulse test. This benefit would only be present in cases with positive result where the use of the caloric testing could be avoided. This assessment comes from diagnostic accuracy studies showing that the video head impulse has high specificity and low-sensitivity when compared with the caloric testing.
Guidelines on benign paroxysmal positional vertigo, Menière’s disease, vestibular paroxysmia and vestibular migraine do not mention this technology. Only one clinical practice guideline on bilateral vestibular disease mentions the video head impulse test as an additional alternative in the diagnostic process. The only coverage policy that mentions video head impulse test does not cover it for Menière’s disease diagnosis.
No economic evaluations assessing this technology have been found.
