[Rapid on-site evaluations of cytological punctures]

Original Title: Evaluación rápida in situ de punciones citológicas
Authors' recommendations: Low-quality evidence suggests that the use of rapid on-site evaluation (ROSE) in fine-needle aspiration of thyroid nodules might reduce the rate of insufficient samples; even though there is no direct evidence showing that this accounts for fewer diagnostic procedures, complications or shorter time for diagnosis. The inconsistency in the rate of insufficient samples might be due to the expertise of the person performing the puncture. Moderate-quality evidence suggests that the use of ROSE, when compared with not using it, in endoscopic ultrasound fine needle aspiration for both pancreatic and mediastinal lesions, does not improve the diagnostic yield or decrease its complications or cost. The use of ROSE in conventional transbronchial needle aspiration of mediastinal lymph nodes might reduce the number of non-clinically significant minor bleedings. Very low-quality evidence does not allow to draw conclusions on the risks and benefits of using ROSE for other indications. The clinical practice guidelines recommend the use of ROSE for thyroid nodules or neck tumors after a previous non-diagnostic cytology. Guidelines on endoscopic ultrasound fine needle aspiration for both pancreatic and mediastinal lesions do not recommend the routine use of ROSE. No coverage policies on this technology have been found. No economic evaluations assessing this technology in the region have been found.
