Authors' recommendations: CONCLUSIONS Low-quality evidence suggests that in patients with advanced ankle osteoarthritis improvement in pain and function of the lower extremity is similar after total ankle replacement or ankle arthrodesis. Also, the patients with ankle prosthesis might have a better gait pattern when compared with arthrodesis patients. The heterogeneity of the results does not allow concluding which treatment is associated to a higher risk of reoperation or complications. No clinical practice guidelines have been found for this condition. One United States scientific society recommends total ankle replacement as an alternative to arthrodesis in cases of advanced osteoarthritis, and in the presence of contralateral ankle arthrodesis and/or osteoarthritis of adjacent joints. Public sponsors from Mexico, Canada, Australia and the United States cover this technology. In Argentina, no studies assessing the cost-effectiveness of this technology have been found.
