Effectiveness, safety and economic evaluation of the existing alternatives for the early detection of Attention Deficit Hyperactivity Disorder (ADHD)

Original Title: Efectividad, Seguridad y Evaluación Económica de las Diferentes Alternativas Existentes en la Detección Precoz del Trastorno por Déficit de Atención con Hiperactividad (TDAH)
Authors' objectives: The aim of this systematic review is to assess the available information related to the efficiency, effectiveness and safety of the existing alternatives for the early detection of ADHD.
Authors' results and conclusions: For the efficency of early detection of ADHD, 210 studies were retrieved, none of which included an economic evaluation of the different alternatives for ADHD early diagnosis. Regarding the effectiveness of early detection of ADHD, 284 were retrieved but any of them assessed wether the screening of ADHD reduce the impairment associated to the disease. No studies that assess what screening strategy is the most cost-effective for the early detection of ADHD have been found. Also no scientific evidence about to what degree early detection improves the morbidity and mortality of ADHD has been identified. On the other hand, experts do not provide strong arguments to make recommendations for or against screening for ADHD. It is still necessary to investigate about the potential benefits and risks of diagnosis and early intervention for ADHD.
Authors' methods: The following databases were searched: PubMed/MEDLINE, EMBASE, Lilacs and the databases of the Centre for Reviews and Dissemination (CRD): NHS Economic Evaluation Database (NHS EED), Health Technology Assessment Database (HTA), Database of Abstracts of Reviews of Effects (DARE). Databases of the following health technology assessment agencies were also searched: CCOHTA of Canada, NICE of United Kingdom, HTA programme of United Kingdom, AUnETS platform of Health Technology Assessment Agencies and Units from the Spanish National Health System. In addition, the databases of the following government agencies and scientific societies were also searched: U.S. Preventive Services Task Force, Asociación Española de Pediatría, Asociación Española de Pediatría en Atención Primaria, UK National Screening Committee, American Academy of Pediatrics (AAP), Centers for Disease Control and Prevention (CDC) and Scottish Intercollegiate Guidelines Network (SIGN). Complete economic evaluation studies were selected for the first research question (efficiency). The second research question (effectiveness) was be focused on cohort and case-control studies. Searches were limited to studies in Spanish, English, French and Italian published until December 2013 and the selection of the studies was done according to previously defined inclusion and exclusion criteria.
