Original Title: Desfibrilador subcutáneo
Authors' objectives: To assess the effectiveness and possible benefits and adverse effects of subcutaneous defibrillators.
Authors' results and conclusions: The studies retrieved and finally considered were heterogeneous, including a broad range of indications for implantation of a subcutaneous ICD. The majority of studies are individual case studies. There are no comparative prospective studies of subcutaneous and intravenous ICDs with sufficiently long follow-up times. The subcutaneous system is not a valid alternative to the transvenous system when permanent stimulation is required. Use of a subcutaneous ICD system is contraindicated in patients with symptomatic bradycardia, continual ventricular tachycardia and patients with documented spontaneous ventricular tachycardia. Conclusions Only short-term evidence regarding the efficacy of inserting a subcutaneous defibrillator to prevent sudden death is available. This evidence indicates that such a device is appropriate as regards short-term safety, although there is some uncertainty concerning its long-term durability. Follow-up data regarding the efficacy of this technique, the durability of the devices used and the need for procedures to check and/or replace defibrillators when necessary should be collected.
Authors' methods: A time-unrestricted search of the scientific literature in databases that collect primary, secondary and applied research studies, namely the Medline, CRD (INAHTA, NHS EED, DARE), Cochrane Collaboration, National Guidelines ClearingHouse (NGC), EuroScan and ECRI databases, in order to locate systematic reviews, clinical practice guidelines or clinical trials that analyse the technique under study.
