[Relationship between use of cannabis and other drugs and psychiatric pathology in adolescents. A proposal of inpatient and outpatient mental health care in the Basque Health Service]

Original Title: Relación entre el consumo de cannabis y otras drogas y patología psiquiátrica en adolescentes. Propuesta de atención en salud mental intra y extrahospitalariade Osakidetza en Euskadi
Authors' objectives: 1. To study psychiatric pathology associated to cannabis and other drugs use in adolescents served in the inpatient and outpatient facilities of the Basque Health Service. 2. Analysis of techniques, therapeutic approaches, organizational design and resources of care in the inpatient and outpatient facilities of the Basque Health Service. 3. To prepare a proposal for a specific intervention program to be eventually set up in the Basque Health Service with the aim of improving the effectiveness of current care to these patients.
Authors' results and conclusions: A tiered program of intervention based on frequency and intensity of use and also on the presence or not of comorbid psychopathology is recommended. Three levels of care are suggested: 1: Occasional use without psychopathological consequences. Educational approach, motivational intervention and treatment of baseline pathology. 2: Use with psychopathological consequences, without dependence. A specific intervention on comorbid disorder plus a more intense intervention directed to use of cannabis or other substances is added. A specific intervention is proposed for adolescents with cannabis use and Conduct Disorders (based on Dialectic Behavior Therapy) or cannabis use and psychotic disorder (Group intervention). 3: Severe pathology related to use (severe abuse or dependence). Intervention in Drug Use Treatment facilities.
Authors' methods: Study of a sample of adolescents treated in outpatient and inpatient Mental Health facilities in the Basque Health Service oriented to the assessment of clinical and sociodemographical data. Comparative analysis between adolescent users of cannabis and other drugs and non-user adolescents with the aim to determine psychopathological characteristics associated to the use of such substances. Collection of information about real care practice in our area addressing adolescents users of cannabis and other drugs. Collection of information on organizational designs in this field. Discussion of results in parallel to a bibliographic review of the subject and elaboration of a report delineating recommendations of specific therapeutic interventions.
