Authors' objectives:
Evaluate the safety and tolerance of online hemodiafiltration (OL-HDF) in comparison to conventional high flux hemodyalisis (HD) techniques.
Determine the effectiveness of OL-HDF in comparison to conventional high flux HD techniques.
Evaluate the importance of the quality of the water used for OLHDF.
Identify those patients that benefit most from treatment with OLHDF.
Estimate the cost of the OL-HDF technique
Authors' results and conclusions:
• OL-HDF does not present any additional safety or tolerance problems in comparison to conventional high flux HD techniques. However, according to available evidence, it should not be concluded that OL-HDF is safer and better tolerated by patients. Future tests should take into account patient acceptability and issues regarding the safety of the liquid used in OL-HDF.
• As regards the effectiveness of this technique, the available evidence indicates that it significantly reduces the risk of death from all causes when compared to HF-HD.
• Differences in terms of quality of life or the nutritional state of patients when comparing the two treatment methods have not been found.
• The available evidence is not conclusive when considering the effects of OL-HDF on hospitalisation rates, the variations in the needs for erythropoietin, blood pressure, speed of growth in children or on dialysis-associated amyloidosis.
• The direct cost per session of OL-HDF for 2016 is 220.77 €. Calculated for the hospital Galdakao-Usansolo (Bizkaia).
Authors' methods:
To respond to the first four objectives, systematic reviews of the scientific literature that responds to these issues have been carried out. To establish the cost of the technique, a financial evaluation has been carried out to calculate the direct costs per session of OL-HDF for two scenarios: one in which the costs were valued in euros in 2013 and another in euros in 2016.
