Follow up of breast cancer in primary care. Safety and efficiency.

Original Title: Seguimiento del cáncer de mama en Atención Primaria.
Authors' objectives: The objective of this report is to assess follow-up of patients after treatment for breast cancer in primary care, in terms of: Effectiveness: mortality, recurrence rates, disease-free survival, satisfaction and health-related quality of life. Safety: delay in diagnosis of recurrence and adverse events. Economic efficiency: cost and quality-adjusted life year.
Authors' results and conclusions: There is limited evidence provided from two studies about the effectiveness and safety of follow up of patients after treatment for breast cancer in primary care. The quality of the evidence varied from moderate to low. Two documents were included in the review that evaluated patient satisfaction with breast cancer follow up. A moderate quality study showed that breast cancer patient satisfaction was better with follow-up in primary care. A low quality study showed better patients satisfaction with hospital care follow up. Patients reported higher degree of satisfaction with the follow-up provided by primary and hospital care. No study was localized that evaluated breast cancer in men or disease-free survival in patients in primary care follow-up for breast cancer. The findings indicate no difference in health related outcomes and effectiveness between primary and hospital care follow up for breast cancer. Localized studies showed no adverse events related to primary care follow up. A single study that was realized in our setting compared the cost of primary care vs. follow up in hospital. Despite limitations, follow up in primary care was less costly.
