Bronchial thermoplasty in patients with severe asthma. Efficacy, safety and efficiency.

Original Title: Termoplastia bronquial en pacientes con asma bronquial severo o grave. Eficacia, seguridad y eficiencia.
Authors' objectives: To assess efficacy (in terms of quality of life, asthma control, number of exacerbations, lung function, medication and days without asthma symptoms), safety and economic aspects of bronchial thermoplasty in adult patients with moderate to severe persistent asthma, and resistant to standard care.
Authors' results and conclusions: The included evidence about the efficacy of thermoplasty in moderate to severe asthma resistant to standard care, showed little improvement in reducing exacerbations, hospital admissions and lost work days, and no significant or clinically relevant differences in the scores of control asthma or quality of life. The evidence about safety, available in short and medium term (5 years), showed this technical as adequate. The evidence about economic aspects was scarce, but indicates that the thermoplasty is a cost-effective alternative. However, thermoplasty may not be considered an alternative to replace another, but rather an added therapeutic step. The inclusion of the technique requires significant costs in the first year of treatment; however it may offer significant savings over the medium and long term, with the appropriate selection of candidates.
